If you are looking to purchase goods in Michigan and you have tax-exempt status, you need to fill out this form and present it to the seller at the time of your purchase. Michigan does not issue tax-exempt numbers, so sellers must have this form in order for you to be granted your tax-exempt status. It is important to note that tax-exempt status is only granted on qualified sales. As a purchaser, it is your responsibility to ensure that both you and your transaction qualifies for tax-exempt status. Seller must keep this form in either digital or paper form for four years following the transaction.
